New Services Available!
At Eclipse Health Services, we're dedicated to providing the best care for all our patients. In an effort to expand our services to a wider audience, we're introducing walk in appointments, primary care, and IV therapy.
Walk In Appointments
As we shift away from practice fusion, all clients are encouraged to come in anytime during office hours for consultations. With several accepted insurances under our belt, it's seamless to get coverage for your needs. If scheduled appointments are preferred, feel free to shoot us an email in our Contact us section, or call 763-340-2330. As for your appointments, there still are some things that are important to remember before you next visit.
For Our Assessments/Diagnostics, Remember to Bring:
- Your Emailed or Mailed appointment sheet with the services we'll be providing.
- A Drivers license, Veteran / Military ID card, or Any other document verifying your identity.
Some Key Tips to Remember:
- We do not provide the results of your assessment, as those are sent to you directly.
- Make sure you've filed for any or all issues you want to be covered, as we will only assess the issues stated in your appointment file.
- Recording/taping sessions is not allowed and will end the assessment immediately.
- If you're not satisfied with the outcome of the claim you've received, reach out to your evaluation company directly for a reassessment. (ie; VES, or SS)
- Feel free to grab the snacks available! they're free :)
Primary Care
Our newest venture has been an exciting journey for all our staff. For those interested in a new primary care physician, getting started is a breeze. For first timers, you'll get your account set up once you arrive at the clinic. We've partnered with Athena One, which will give you a private link to finish setting up your account following your first appointment. After that, setting up appointments is as easy as logging in and choosing a day that works best for you!
IV Therapy
Finally, our last newest addition, IV therapy. Stop by and inquire with our front desk or physician on the different types of IV available, take a seat in our cozy therapy room and relax as your treatment does it's magic. Our clinic co-resides with Zoëlife aesthetics and wellness, So if you're curious on other aesthetic opportunities available, feel free to ask!
